Request a refill before medicine runs out

9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For young adults who take a daily medicine and whose family now notices when it is running low. Fits students who use a phone or calendar; the family and nurse agree on the refill plan first.

Baseline
Ask the family how the last few refills happened and who started them. Record each refill point as started by the student without a reminder, started after a reminder, or missed.
Tips
Running out of a medicine can be a health risk, so the target is every refill. Teach the student to read the refills-left line on the label; zero refills means calling the doctor's office, which can join the appointment goals.
Limits
Gives no medical advice and does not change who manages the prescription. Insurance and prior-authorization problems are handled by the family and pharmacy.

Goal

By the annual IEP review date, each time a daily medicine reaches the refill mark (such as seven days left), with the pharmacy app or phone number saved in the student's phone, the student will start a refill request through the pharmacy app or phone line before the medicine runs out independently in 4 of 4 opportunities, across 4 consecutive refill cycles, as measured by direct observation collected at each refill point, by family or nurse report.
