Bring needed items to each appointment

9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For students who get to appointments but arrive without their ID, insurance card, forms, or questions, so a family member has to fill in. Present levels in planning ahead decide fit, not grade.

Baseline
Track 3 to 5 appointments. Score correct only when every needed item is with the student on arrival without a same-day reminder.
Tips
Keep a photo of the ID and insurance card in the phone as a backup, with the family's agreement. A list of questions for the provider builds self-advocacy at the same time.
Limits
Appointments are spread out, so data build slowly. The family decides which documents the student carries.
