Name trusted supporters for each life area

When to use this goal

For students nearing the end of school who rely on one adult, often a parent or teacher, for every decision. Present levels in communication decide whether the map is written, drawn or built with photos.

Baseline
Ask the student to fill in the map without the contact list and score it. Note whether supporters named are real and reachable (a current phone or email), not only a name.
Tips
The map belongs to the student and goes home; families often add supporters the school does not know. Use it with tn-sd-008 so the named supporters are the ones the student practices deciding with. Add adult service contacts from fn-tr-014 as they become known.
Limits
Naming a supporter is not the same as using one. The map can go out of date quickly, so update it before each IEP meeting.
