Decide when a symptom needs a doctor

9-12, 6-8OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For older students moving toward managing their own health who do not know the difference between home care, a doctor visit, urgent care, and 911. Present levels, not grade, decide fit. Plan the key with the school nurse.

Baseline
Run six role-plays with a mix of levels and note the student's choices. Check that the student knows their own doctor's name and how to reach them.
Tips
Each scenario is one opportunity, scored against the nurse's key. Use the student's real doctor, insurance card, and phone contacts where possible.
Limits
Role-plays may not reflect real decisions when the student is unwell. Not medical advice; the nurse and family guide each student's plan.

Goal

By the annual IEP review date, during health class role-plays of symptom scenarios, the student will choose the right level of care for the scenario and say who they would contact and what they would say independently in at least 5 of 6 opportunities, on 3 of 4 consecutive weeks, as measured by opportunity-by-opportunity data collected weekly during the unit.

Baseline: 2 of 6 opportunities.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Opportunity (trial) recording gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Health Education.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.
