Fit shapes into a form board

PKOpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For preschool students who push shapes at the board without turning them to fit, which shows early spatial orientation needs. Fit depends on a trial with a form board, not age.

Baseline
Offer the 3-shape board and then the 5-shape board and score each shape fit within about 30 seconds. Note trial-and-error turning versus planned turning.
Tips
Knobbed boards help students with weak grasp. Turning the board tests whether the student uses shape, not memory of the spot.
Limits
A narrow task; carryover to puzzles (mt-vm-012) should be checked.

Original IEP Atlas template based on general measurement principles; not copied from any goal bank. ID mt-vm-035.

Criterion

Number of opportunities in which the behavior is demonstrated, out of a fixed number.

Sample numbers. Replace them with the student's data.

Data collection

Each planned or naturally occurring opportunity is recorded as correct/independent, prompted, or not demonstrated.

Goal

By the annual IEP review date, given a form board with 5 shapes including a star and an oval, the student will turn each shape to match its space and fit it into the board independently on 5 of 5 items, across 3 consecutive weekly sessions, as measured by opportunity-by-opportunity data collected weekly.

Baseline: 2 of 5 opportunities.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Opportunity (trial) recording gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Visual-Motor & Visual-Perceptual Skills.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.

Short-term objectives (optional)

When are objectives required?
