Report on a home goal set with family

When to use this goal

For students whose goal work happens only at school and who are ready to carry it home. Present levels in communication decide whether the report is spoken, written or shown with photos from home.

Baseline
Track the first month of weekly check-ins as baseline. Talk with the family about how realistic the log is at home; a log that is never filled in is a planning problem, not a student failure.
Tips
The family and student choose the home goal; school only supports the check-in. Keep the log short. Families who cannot track can send a photo or a one-word text instead.
Limits
Relies on family participation, which varies with family schedules; never penalize the student for that. Measures reporting, not whether the home goal was reached.

Goal

By the annual IEP review date, at a weekly check-in, after setting a home goal with family, such as making own lunch or practicing an instrument, with a home log, the student will report progress on the home goal using the home log and name the next step independently in at least 3 of 4 opportunities, across 2 consecutive months, as measured by opportunity-by-opportunity data collected weekly.

Baseline: 1 of 4 opportunities.
